Wellbeing for Life

Wiltshire’s 5 Year Plan for health & social care
NHS Wiltshire Clinical Commissioning Group (CCG) is keen to hear your views and get your input into the provision of healthcare services across Wiltshire over the next five years.
Facilitated by Avril Baker Consultancy, the CCG is undertaking a consultation exercise aimed at identifying some of the barriers or problems that patients and the public may have experienced in accessing or receiving healthcare and feedback on future priorities. Feedback from local groups will enable the CCG to plan and organise better delivery of healthcare in the county over the next five years through progressing the following aims:
  • Encourage and support individuals to take responsibility for their own well-being
  • Ensure more equitable access to a high quality and affordable system, which delivers the best outcomes for the greatest number
  • Enable better provision of care at, or close to home, thereby reducing unnecessary hospital admissions

If you have any questions about this consultation please contact the Communications Team at Wiltshire CCG on 01380 733738 or e-mail wccg.info@nhs.net
